PREP CHECKLIST

What to bring.

Everything below, including your completed new patient questionnaire, must be sent to reception a minimum of 3 days before your appointment. This gives your doctor time to review your history and prepare. Items received less than 3 days before your appointment may not be reviewed in time.

01

Completed questionnaire including current medications and supplements with doses, and a note of your main concerns

02

Recent blood tests, medical reports, hospital paperwork, specialist letters — even tests reported as normal

03

Previous imaging

04

Any prior functional or specialist testing

THE FLOW

In the room and after.

What the appointment itself looks like, and what happens once you leave.

DURING THE APPOINTMENT

A comprehensive history and clinical review covering symptoms, medical and surgical history, family history, medications and supplements, lifestyle (sleep, stress, diet, exercise, work), hormonal and menstrual history where relevant, and a focused physical examination where relevant. You leave with a clear testing plan, practical recommendations to start now, and a scheduled follow-up to design your treatment plan.

AFTER THE APPOINTMENT

Testing is ordered through preferred functional labs — some at home, some need a blood draw. Most results return in 7 to 14 days. Follow-up consultation when your treatment plan is built (15 to 60 minutes depending on complexity). Most patients see us several times in the first 6 to 12 months. Telehealth available for existing patients.

You will not leave with a complete diagnosis on day one. That is by design. The first appointment maps the picture and orders the testing. The treatment plan is built at follow-up, once the testing has come back.

Questions we hear often.

Do I need a GP referral?

No. Book directly. We work alongside your GP, not in place of them, and we can write to your GP with our findings and recommendations where useful.

Do you bulk bill?

No. Mojo Klinik is private billing. Medicare rebates apply for eligible in-person initial consultations.

What about telehealth and Medicare rebates?

Telehealth (Zoom) is available Australia-wide for all appointment types. For your first appointment, in-person at our Neutral Bay clinic is strongly recommended. A first telehealth appointment is not eligible for a Medicare rebate, and some clinical options may not be available. Once seen in person, you remain telehealth-eligible for follow-ups for the next 12 months.

What is the cancellation policy?

48 hours notice is required. Cancellations or reschedules within 48 hours of the appointment time are charged the full consultation fee.

How do I book?

All new patient appointments are booked online via our online booking system. Reception cannot take new patient bookings by phone but can be reached on 02 9133 8500 for general enquiries.
